ECA Athletics Forms
NCHSAA Athletic Participation
The North Carolina High School Athletic Association requires that all students trying out or participating in any sport complete the Concussion Protocol, Physical and COVID-19 Questionnaire forms prior to tryouts. Beginning 2025 – 2026 it is now required that these forms be submitted through Dragonfly which organizes digital records, health forms and communication.
Dragonfly Instructions

Instructions for Dragonfly:
- Visit Dragonflymax.com and click the “Log-in/Sign-Up” button.
- Select “Sign up for free” then follow the steps to create your Parent account with your email address.
- Verify your account with the verification ID sent to your email address.
- Click “Connect to your school” to select “Parent” as your role and select your child’s school.
- After selecting your child’s school, click “Join” to request access. An administrator at your school will approve your request.
- Click “Set up your children” and follow the prompts to add your kid(s) and fill out their participation forms.

- Visita Dragonflymax.com y haz click en el botón “Log-in/Sign-Up”.
- Selecciona “Sign up for free” y sigue los pasos para crear tu cuenta de Padre con tu correo electrónico.
- Verifica tu cuenta con el código de ID que fue enviado a tu correo electrónico.
- Haz Click “Connect to your school” para seleccionar “Parent” como tu título y selecciona la escuela de tu hijo.
- Después de ver la escuela de tu hijo, haz click en “Join” para pedir acceso. Un administrador de la escuela lo aprobará.
- Haz Click en “Set up your children” y sigue las instrucciones para agregar tu estudiante(s) y llena el formulario de participación.
